Rebuilding Public Health Systems in an Era of Converging Crises: A Climate-Sensitive Resilience Framework

Abstract
Background: The COVID-19 pandemic exposed deep structural weaknesses in public health systems across the world, including fragmented surveillance, fragile service capacity, inconsistent risk communication, and persistent health inequities. At the same time, the climate crisis is intensifying heat-related illness, vector-borne disease, extreme weather disruptions, displacement, and pressures on health infrastructure and workforces. Contemporary health threats are increasingly converging rather than isolated.
Methods: Drawing on a conceptual synthesis of COVID-19 lessons, health system resilience scholarship, and recent guidance from the World Health Organization and the IPCC, this article develops the Integrated Climate-Sensitive Public Health Resilience (ICPHR) Framework, a four-dimensional and system-based model organized around preparedness, absorption, adaptation, and recovery.
Results: The framework conceptualizes resilience as a dynamic and cyclical process through which public health systems anticipate, withstand, learn from, and transform after shocks while maintaining essential functions. The analysis identifies five strategic pathways for resilient system transformation: integrated surveillance and early warning, stronger primary health care, trust-based risk communication, equity-centered design, and cross-sectoral governance.
Conclusions: Resilience in the coming decade must be climate-sensitive, institutionally embedded, and oriented toward long-term system transformation rather than temporary emergency response. Unlike existing models, the proposed framework explicitly integrates climate-sensitive risks into a cyclical and system-based structure. This framework provides actionable insights for public health system strengthening, particularly in integrating pandemic preparedness with climate-sensitive health risk management. Public health resilience should become the central paradigm guiding health system transformation in an era of converging global crises.
